Points Review Application Form
- According to the Award Criteria Guidelines for Junior High School Graduation Ceremony at NNKIEH(國立南科國際實驗高級中學國中部畢業典禮學生受獎標準實施要點).
- Technology Award (Xinshi District Award) Criteria:
- One student per class, with a dual-track application process consisting of teacher recommendations and parent self-nominations.
- The student must have demonstrated excellent academic performance during their time at school, with outstanding achievements in mathematics, science, or technology, serving as a role model for their classmates.
- The final score is calculated by taking 30% of the total grades in the mathematics, science and computer science and 70% from other outstanding achievements. (In reference to the Junior High School Mayor’s Award Criteria 國中部應屆市長獎給獎實施細則)
- Language Award (Shanhua District Award) Criteria:
- One student per class, with a dual-track application process consisting of teacher recommendations and parent self-nominations.
- The student must have demonstrated excellent academic performance during their time at school, with outstanding achievements in language-related fields (such as Mandarin, English, local languages, or other languages), serving as a role model for their classmates.
The final score is calculated by taking 30% of the total grades in language-related fields and 70% from other outstanding achievements. (In reference to the Junior High School Mayor’s Award Criteria國中部應屆市長獎給獎實施細則.)
★ Submission Deadline: Please submit this application form along with copies of certificates/awards and relevant event materials (A4 size) to Mr. Alvis, by 11:59 AM May 11, 2026.
Notes:
- Each student may have a maximum of 3 special achievements recognized per academic year, with a total of 6 special achievements counted throughout the three years of junior high school.
(Supporting documents should be arranged in chronological order by academic year and secured with a binder clip.) - Team awards will be calculated at half the individual competition score.
- International competitions must have at least six participating countries (teams/individuals), and national competitions must have at least six participating counties/cities (teams/individuals). Supporting documents proving competition results must be provided for recognition. Competitions organized by private entities must have government approval documentation to be considered. School-level competitions will not be counted.
Students who have previously applied for the Mayor’s Special Award are not required to resubmit your application materials and will automatically be considered as applicants for this District Mayor’s Award. However, students or parents may resubmit materials if supporting documents need to be updated or if applying for a different award category (for example, previously applying for the Arts Award and applying for the Technology Award this time).
