★ School Influenza Vaccination Date: October 23, 2025 (Thursday)
★ Deadline for Online Consent Form Submission: September 19, 2025 (Friday)
(To avoid repeated reminders from IBST Office, please make sure to complete the signing process before the deadline.)
Please complete the consent form online :

★Please sign in clear block letters.
★ Parents may rescan the school QR code or click the link to modify their consent form before the deadline (September 19). Each new submission will overwrite the previous one, and the final submission will be considered valid.

★ On the day of vaccination, students must bring their National Health Insurance Card. Students without the card will not be able to receive the vaccine.
★ All students must complete the consent form, whether or not they intend to receive the vaccination.
★ No on-site, last-minute consent will be accepted on the vaccination day (G1-G6 Oct.17, G7-G12 October 23).

Please note:

The influenza vaccine is provided free of charge through the Public Health Bureau. The vaccine brand cannot be chosen. Parents who have concerns about vaccine brands should select “ Disagree ” on the form.

Students who checked “ Agree ” but do not receive the vaccine at school on the scheduled date, as well as students who checked “ Disagree,” will receive a make-up vaccination notice within one week after the school vaccination day.

With the notice, students may go to designated health centers (no registration fee required) or contracted medical institutions for vaccination, but will need to pay related medical service fees.

To prevent duplicate vaccination, public influenza vaccines will not be administered without the official make-up notice.
