說明： 一、依據教育部國民及學前教育署 111 年 11 月 1 日臺教國署學字 第 1110151810 號函辦理。

            二、依據本年 10 月 27 日「衛生福利部傳染病防治諮詢會預防接 種組(ACIP)」第 8 次專家會議決  議，經評估疫苗臨床試驗安 全性與有效性結果、國內外疫情風險，及參酌國際間追加 劑接種政策、衛生福利部食品藥物管理署核准情形，建議 Moderna (Spikevax) COVID-19 雙價疫苗(原病毒株 /Omicron 變異株 BA.1)實施對象擴及 12 歲至 17 歲青少年追加 劑接種，包含尚未接種追加劑及已完成單價疫苗追加劑接 種者。

           三、旨揭疫苗接種作業自本年 11 月 1 日起實施，追加劑與最後一 劑基礎劑、基礎加強劑或前一劑追加劑間隔至少 12 週(84 天)。請學校於疫苗接種前加強衛教宣導說明。基於青少年接種 mRNA 疫苗後可能出現的反應及罕見且多屬輕微之心肌 炎或心包膜炎，請家長/監護人詳閱接種須知，瞭解接種後 可能發生的常見副作用及應注意事項後，填寫「滿 12 歲至 17 歲接種評估及意願書」，經醫師評估後接種，更新版之 「莫德納(Spikevax) COVID-19 雙價疫苗接種須知暨接種評 估及意願書」如附件。

           四、另請提醒家長/監護人於完成接種後，可透過接種單位提供 的海報或須知上的 QR code 掃描加入 Taiwan V-Watch 疫苗接 種-健康回報，藉由家長/監護人關注子女回報接種後狀 況，提升對 COVID-19 疫苗安全監測、相關關懷照護及因應 效率。

           五、此外，對於民眾「經醫師評估不適合接種 mRNA 疫苗者」或 「希望選擇其他種類疫苗接種者」等對象，可選用國內核 准之單價疫苗作為追加劑使用，爰於上述情形 12 歲至 17 歲 青少年可選用之疫苗包括 BioNTech 及 Novavax 等。

          六、本府衛生局將妥為安排旨揭疫苗接種相關方式及作業，如 有評估規劃校園集中接種，再另行通知學校。

         七、檢附「莫德納(Spikevax) COVID-19 雙價疫苗接種須知暨接 種評估及意願書(更新版)」1 份，相關接種資訊亦可透過衛 生福利部疾病管制署全球資訊網「COVID-19 疫苗接種院 所」(https://reurl.cc/7eW0AQ)，利用「COVID-19 疫苗接 種院所資訊」或「COVID-19 疫苗接種院所地圖」，就近選 擇合適地點接種，以增進民眾接種可近性。
